Transforming Diabetic Services in Primary Care Using Population Health Management (PHM)
This is a story of service transformation.
It is also a Population Health Management (PHM) story centred in Primary Care. It starts with General Practitioners (GPs) undertaking a critical review of an existing chronic disease management service; in this case diabetes. The clinicians accepted that they could do much better for the diabetic population that they served.
Armed with novel and actionable insights provided by Sollis the diabetes service for the Kingston Health Centre is currently being transformed from a reactive to a more proactive, anticipatory and personalised service; a service with the patient at the centre.
It is a service where decision making is truly data driven.
