Sollis Clarity Population Health Analysis for NHS Wales

We are delighted to announce our partnership with the Cwm Taf University Health Board to help them pilot their population health management strategy, based on analyses using Sollis Clarity. The health board’s pilot project is using Sollis Clarity’s comprehensive set of risk models (including the Johns Hopkins HealthCare ACG® System) to better understand the patterns of multimorbidity across their segmented populations. Our health analytics are helping to identify areas of unwarranted variation so that the health board can design, roll out and monitor best practice care models, targeting patients for intervention and monitoring and evaluating the outcomes. The pilot will run for nine months, allowing the health board to fully assess the benefits of our population health management software and services.

“We are delighted to be working with Sollis and with our Rhondda Cluster of GP Practices to pilot our particular approach to population health management in Cwm Taf. It is our expectation that this work offers us novel and actionable intelligence on the patterns and determinants of health states in our population. This should enable us intervene more smartly to improve population health outcomes and enhance the quality of care we deliver to our population, achieving both at increasingly lower cost per capita.”
— Dr. Kelechi Nnoaham
Executive Director of Public Health, Cwm Taf University Health Board

This project follows our previous work in Wales for the Abertawe Bro Morgannwg University (ABMU) Health Board through funding from the Small Business Research Initiative.
