We are delighted to announce our partnership with the Cwm Taf University Health Board to help them pilot their population health management strategy, based on analyses using Sollis Clarity. The health board’s pilot project is using Sollis Clarity’s comprehensive set of risk models (including the Johns Hopkins HealthCare ACG® System) to better understand the patterns of multimorbidity across their segmented populations. Our health analytics are helping to identify areas of unwarranted variation so that the health board can design, roll out and monitor best practice care models, targeting patients for intervention and monitoring and evaluating the outcomes. The pilot will run for nine months, allowing the health board to fully assess the benefits of our population health management software and services.
